2025 1095C Notification
Goodwill Greater Milwaukee and Chicago wants to ensure employees and former employees know how to access their 2025 Form 1095-C, information about the health insurance coverage offered to them during the year. Form 1095-C helps verify your health coverage. You should keep your 1095C form with your tax records. Tax related questions about your 1095C should be directed to your tax advisor.

How to Access Your 2025 1095C Electronically
- Log in to the Empyrean employee portal using your credentials www.goodwillsewbenefitsenroll.com
- Click on MENU in the upper right corner
- Click on the down arrow next to FORMS
- Click on 1095 FORMS MANAGEMENT
- A box will pop up
- Click on VIEW AND DOWNLOAD YOUR FORMS
- Click on the eye next to the year you would like to view
How To Request a paper copy of your 2025 1095C form

Former Goodwill Employees
A paper copy of your 1095C will be mailed to the last address on file with Goodwill. If you need to update you address, contact the Employee Service Center at 414-977-4914 or email [email protected].
